Care Coordinators

Care coordinators play an important role within a Primary Care Network.

They proactively identify and support patients, including the frail/elderly and those with long-term physical and mental health conditions. The team provides coordination and navigation of care and support across health and care services for those patients. The care coordinators ensure that patient health and care planning is timely, efficient, and patient-centred.

The Care Coordinators role has a wide scope and is often used differently from PCN to PCN. Tunbridge Wells PCN Care Coordinators work with patients across several areas including our Care Homes, Cancer, Health Inequalities, patients with Learning Difficulties and general education for the PCN.

They often work as part of a Multi-Disciplinary Team (MDTs), where teams of various health and care professionals meet to discuss patients with complex healthcare needs, to ensure that all the patient’s needs are met by the most appropriate team in a timely way.
