Primary Care Liaison Learning Disability Nurse

Lisa is a Learning Disability Nurse working across the Tunbridge Wells PCN.

All patients with a learning disability from the age of 14 are entitled to an annual health check at their GP Practice.

If you have a learning disability and would like a health check, please contact your surgery to book an appointment.

You will be offered a longer appointment with Lisa first.

Lisa will give you time to say what is important to you about your health. Lisa will work with you to make sure any reasonable adjustments are in place so you can access the health care you need.

Lisa will complete a Health Action Plan with you so that you have a record of any actions agreed at the appointment.

You will usually have a shorter appointment with the GP afterwards if needed.
